Care Transition Coordinator

Care Transition Coordinator Job Description– Care Transition Coordinator (ID 37782, 37783; 26141351) The Montana Department of Public Health and Human Services (DPHHS) is offering a position in the Behavioral Health and Developmental Disabilities Division, Treatment Bureau, within the Care Transitions section. This role supports the implementation of a Critical Time Intervention (CTI) program for low‑income individuals with serious or disabling mental illness who are transitioning from institutional settings to community living. Responsibilities Research, develop, and implement CTI program components. Coordinate care for individuals during major life transitions. Manage multiple complex cases for low‑income SDMI patients. Establish and maintain community partnerships to facilitate access to services and supports. Collaborate with diverse providers, staff, and subject matter experts. Qualifications Knowledge of the characteristics and symptoms of serious or disabling mental illness in adults. Strong analytical and critical thinking skills. Experience managing timelines and logistics for complex cases. Ability to interact with diverse groups, providers, staff, and subject matter experts. Bachelor’s degree in a related human services field. At least two years of experience in health, behavioral health, human services, or education field. Other combinations of education and experience may be considered on a case‑by‑case basis. Additional Information Partial telework may be available within Montana; conditions will be defined by supervisor and must follow state policy. Statutory $1.00 per hour base pay increase effective July1,2026. Travel: up to 50 % of the time, normally during working hours. Open until filled, with frequent screening of applicants. May be eligible for a training assignment, subject to agency pay rules.
