Program Manager of Pharmacotherapy (PGY1/PGY2) - Brooklyn

Position Summary We have an exciting opportunity to join our team as a Program Manager of Pharmacotherapy - Brooklyn. In this role, the successful candidate is responsible for creating and managing a specialized pharmacotherapy clinical program within an oncology team. The role works under minimal supervision to troubleshoot pharmacotherapy challenges and manage and supervise all pharmacotherapy needs in an oncology pharmacy clinical program. The candidate is responsible for creating, implementing, and managing a clinical pharmacotherapy program with particular focus on participation in a collaborative drug therapy management (CDTM) agreement. Job Responsibilities Manage a clinical pharmacotherapy program within an oncology team through multidisciplinary rounds and patient clinic visits, provide pharmacotherapeutic, pharmacokinetic, and pharmacoeconomic consultations, and facilitate transitions of care as needed; offer medication education counseling to patients and caregivers; participate in educational initiatives in both inpatient and outpatient settings. Develop, implement, and manage a CDTM agreement with physician providers in an assigned hematology/oncology setting in accordance with state and institutional guidelines. Evaluate and report measures of patient encounter productivity and identify methods to enhance efficiency of such encounters. Maintain proficiency in USP Chapter compounding procedures for emergent clinic-based compounding. Actively participate in tumor boards for select hematology/oncology clinical practice areas. Provide oversight for quality assurance of clinical pharmacy programs to improve quality of care, focusing on creation, review, and management of oncology Epic treatment plans and pathways. Participate in and manage patients within an ambulatory Antimicrobial Stewardship program. Contribute to professional development of staff, including clinical pharmacists. Create and manage practice guidelines to enhance pharmaceutical care and medication safety for hematology/oncology patients. Document and report clinical interventions, adverse drug reactions, and medication errors. Participate in enterprise committees including Pharmacy & Therapeutics, Medication Utilization Evaluation, Cancer Center Clinical Services, and select hematology/oncology medical committees. Preceptor pharmacy residents, mentor pharmacy students, and support scholarly activity. Identify and provide expertise for Medication Use Evaluations and monograph authoring for P&T requests. Maintain professional competency and certification. Support high‑risk oncology patients through rotating weekend on‑call clinical coverage and surveillance of patient charts across PCC sites. Minimum Qualifications Doctor of Pharmacy degree from an ACPE accredited school of pharmacy. New York State Pharmacist licensure with current NYS registration. PGY1 and PGY2 pharmacy residency training. Board certification in Oncology pharmacy (BCOP) within 12 months of hire. Collaborative Drug Therapy Management certification within 12 months of hire. Self‑motivation, good communication, organizational, and interpersonal skills. Preferred Qualifications 2 or more years of post‑residency oncology clinical pharmacy practice experience after PGY2 completion. Experience working in an antimicrobial stewardship program. Qualified candidates must be able to effectively communicate with all levels of the organization. Salary Salary range: $121,792.21 – $190,000.00 annually. Actual salaries depend on experience, specialty, education, and hospital need. The range does not include bonuses, incentive pay, differential pay, or other compensation or benefits.
