Job Summary:

We are seeking a compassionate and dedicated Home Caregiver to join our team at SYNERGY HomeCare of the West Valley in Phoenix, Arizona. As a Home Caregiver, you will provide high-quality, personalized care to our clients, enabling them to live comfortably and independently in their own homes. Responsibilities:

• Provide one-on-one care and support to clients in their homes, including assistance with daily living activities, such as bathing, dressing, and grooming
• Assist with medication management, including administration and monitoring
• Prepare healthy meals and snacks, as needed
• Assist with light housekeeping, laundry, and other household tasks
• Promote client independence and autonomy, while also ensuring their safety and well-being
• Develop and maintain positive relationships with clients, their families, and caregivers
• Document all care provided, including medications, treatments, and client progress
• Collaborate with other healthcare professionals to ensure seamless care coordination

Requirements:

• High school diploma or equivalent required
• Ability to lift, bend, and stand for extended periods
• Occasional lifting of 25 pounds or more may be required
• Reliable transportation and valid driver's license (if required)
• Ability to work varied shifts, including evenings, weekends, and holidays
