POSITION SUMMARY

The primary function of the Care Coordinator is to provide care coordination, in accordance with CCBHC certification criteria, for persons served and their families. The Care Coordinator is responsible for outpatient primary care screening and monitoring, gathering vital data to determine health care needs, providing support and connecting individuals with community resources throughout the course of treatment. This position also serves as a member of the individual’s care team, collaborating with a variety of providers throughout the community. The Care Coordinator is committed to providing high quality service and promoting easy access to care, including transitions, at all levels. This also includes administration and collection of data, using the NationalOutcomesMeasures(NOMs) tool, as required. This position requires an individual to be adaptive, organized, self-motivated and professional with a wide range of people in a variety of settings. Care Coordinators are expected to work both independently and collaboratively and demonstrate strong verbal and written communication skills.

ESSENTIAL FUNCTIONS

Provision of care coordination services to individuals and families, based on assessed needs. Ongoing collaboration with treatment team members, including individuals, families, direct care providers and community partners throughout the course of treatment. Facilitatecollateralcommunicationbetweenpersons served,supportstaff,directcareproviders,andthe community to achieve desired outcomes. Facilitate and support persons served to ensure successful transitions throughout the course of treatment. Develop and maintain collaborative relationships within agency departments and in the community setting, through participation in activities and events. Administer the NOMs , Social Drivers of Health Screening, Health Screening at admission and other specified intervals. Assessment and documentation of vitals at admission and other specified intervals. Documentation of all assessment datainaccordance with program expectations . Collaboratewithall treatment teammembers regular Ensure persons served haveallnecessaryinformationtocompletenextstepsintreatmentpriorto leaving the building after initial appointments. Activeandtimelyparticipationinalladministrativeandtreatmentteammeetingsforpurposeof case review, case assignments, and individual planning. Documentationistimely,current,andaccuratelyreflectsservice provisionandresponse to those services. Actasanadvocateforpersons served.Thisincludesidentifyinggapsanddeveloping proposals for meeting needs and accessing resources and services. Work as part of an integrated treatment team collaborating with the individual served, family members or others they want involved in their treatment and other treatment team members (internal and external) to help support the individual in their treatment and recovery. Demonstrate cultural and linguistic competence. Approach interactions with individuals served and colleagues with a trauma sensitive lens Provide services that are recovery oriented, and person and family centered, utilizing a strengths-based approach. Provide services to meet the whole health needs of individuals served. Utilize appropriate assessment instruments to help monitor the progress of individuals served and ensure necessary services are received in the appropriate intensity and duration. Utilize Evidence-Based Practices (EBPs) as appropriate. Respectanddemonstrateabilitytomaintainthe confidentiality of persons served. SupportandactivelypromotethevisionandmissionofFamilyServiceandGuidanceCenterand the philosophy of the strengths perspective. OtherdutiesasnecessaryorrequiredforthefulfillmentoftheCCBHCprogram,SAMHSAgrant, or other strategic initiatives. POSITION QUALIFICATIONS

Education/Licensure: Bachelor’s Degree in human services field or be equivalently qualified by work experience or a combination of work experience and education. High school diploma required. Must successfully pass the KBI, DCF Central Registry and all background investigations. Must possess a valid state of Kansas Driver’s license, a good driving record, current auto insurance and reliable transportation. Experience: Familiarity with the Community Mental Health System in Kansas and admissions preferred. Experience working with vulnerable/marginalized populations preferred. Experience with interviewing and the intake process preferred.
