This position plays a central role in implementing statewide HCF strategies, coordinating with local partners, and integrating chronic disease prevention initiatives into county-level planning and community programs. The HCF Liaison works collaboratively with state agencies, nonprofit organizations, businesses, schools, and community stakeholders to help Florida residents make informed, consistent choices that promote healthier lifestyles. The incumbent ensures alignment with the state's workplan requirements and leverages existing resources to maximize reach and community impact. This position also coordinates the County Health Department's community operations and outreach logistics to enhance access to public health programs, improve community engagement, and ensure consistent health messaging.The Outreach Coordinator is responsible for coordinating but not personally representing the department at all outreach events unless directed by leadership. The position ensures appropriate staffing, event logistics, strategic messaging direction, and required documentation through the department's Event Tracker App. Enhances community engagement through the Wellness Wave initiative by building partnerships, connecting residents to essential services, and promoting long-term health and resilience. The role also maintains the Health Education and Promotion (HEP) inventory and collaborates closely with the Public Information Officer (PIO) to ensure alignment of public-facing materials with approved communication standards.Outreach Coordinator Responsibilities:Event & Outreach CoordinationPlans, schedules, and coordinates outreach and community events.Completes event registrations on behalf of the CHD.Assigns staff, confirms readiness, and verifies attendance.Manages event logistics, including equipment and vehicle requests.Strategic Message DirectionIdentifies key public health topics and priorities to guide strategic messaging.Collaborates with the PIO on approval of new or sensitive messages.Provides staff with high-level messaging direction rather than detailed talking points.Event Tracker App Documentation & ReportingDocuments all outreach activities using the Event Tracker App.Ensures accurate, timely entry within required timeframes.Produces monthly and quarterly reports on outreach activity.Community Partnership DevelopmentBuilds, maintains, and supports community partnerships.Coordinates participation with partnering organizations.Sends outreach announcements to partners and internal programs.HEP Inventory ManagementMaintains Health Education & Promotion inventory and storage.Conducts quarterly audits and ensures materials are fully stocked.Ensures all materials are current, accurate, and PIO-approved.Internal Coordination & Program SupportSends outreach announcements to Clinic, School Health, WIC, and applicable teams.Attends program meetings and provides updates as needed.Staff Training & DevelopmentTrains staff on outreach procedures and Event Tracker App usage.Ensures staff are fully prepared for assigned events.Quality Improvement & ComplianceTracks outreach metrics and identifies opportunities for program improvement.Ensures compliance with departmental standards and expectations.Wellness WaveSupports the Wellness Wave initiative by coordinating outreach activities that increase community awareness, participation, and access to Department of Health wellness and education programs.Builds and maintains partnerships with local leaders, educators, and health professionals to expand resource availability and strengthen community trust.Facilitates engagement efforts that promote prevention, empower individuals with knowledge, and elevate wellness as a shared community priority.Oversees the connection of residents to essential services and ensures initiative activities align with long-term and resilience goals.Healthy Communities of Florida (HCF) ResponsibilitiesAwareness & Resource LinkingAttend statewide training on chronic disease prevention programs.Share resources from Florida Department of Health programs with partners and the community, including:Asthma Home Visiting ProgramNational Diabetes Prevention Program (National DPP)Diabetes Self-Management & Education Support (DSMES)Epilepsy Services / Epilepsy Medication AssistanceHeart Health PlusInsulin Distribution ProgramPSE Strategies — Nutrition & Physical Activity in Community SettingsImplement one of the following:PACE-EH: Utilize the Protocol for Assessing Community Excellence in Environmental Health to identify a community issue related to food access, physical activity, or the built environment and incorporate findings into county health planning efforts.Local Promising Practice: Implement a SMART nutrition or physical-activity-focused policy or environmental strategy in a community setting.Required Knowledge, Skills, and Abilities:Knowledge of public health principles, community outreach practices, and health promotion strategies. Knowledge of event coordination logistics, including staffing, scheduling, equipment management, and operational planning. Knowledge of communication and health-messaging standards, including working within PIO guidance and approved public-facing materials. Knowledge of data entry, tracking, and reporting systems, including documentation requirements and quality assurance practices. Knowledge of community partnership development and methods for engaging organizations, agencies, and stakeholders. Knowledge of inventory management principles, including auditing, material control, and resource allocation. Knowledge of quality improvement concepts and compliance standards in a public health setting.Strong organizational and time management skills, with the ability to manage multiple simultaneous tasks and deadlines. Skill in coordinating outreach events, managing logistics, and ensuring successful operational execution. Skill in effective communication, including strategic message identification and collaboration with the Public Information Officer. Skill in documenting activities accurately and consistently using digital applications such as the Event Tracker App. Skill in building and maintaining community partnerships, including outreach to partner organizations and internal programs. Skill in maintaining and auditing Health Education and Promotion materials to ensure accuracy and compliance. Skill in preparing reports, tracking metrics, and identifying trends or improvement opportunities. Skill in training staff, conveying procedural information clearly, and ensuring readiness for outreach activities. Ability to work collaboratively with internal programs, external partners, and leadership while maintaining professionalism and accountability. Ability to identify public health priorities and align messaging, outreach efforts, and resources accordingly. Ability to adapt to changing outreach needs, schedules, and operational demands while maintaining accuracy and timeliness. Ability to assess outreach performance, evaluate documentation, and ensure compliance with departmental standards. Ability to develop constructive relationships with community partners to strengthen engagement and access to public health services. Ability to maintain confidentiality and adhere to departmental privacy and communication protocols. Ability to represent the department's mission and values through coordination, communication, and support of public-facing initiatives.Qualifications:This position requires daily in-office or field work. Preferred – A bachelor's degree from an accredited college or university and two years of professional experience in planning, evaluation, research, analysis, business, government, social health, or rehabilitation services or fiscal work. A master's degree from an accredited college or university can substitute for one year of the required experience. A doctorate from an accredited college or university can substitute for the required experience.
