Social Service Coordinator - Bronx Family Housing

Job Title
Provide direct social services and resident management related functions to tenants. Duties include a variety of case management services such as intake and admission related activities, assistance to address a full range of tenant needs such as mental health and substance abuse related interventions, referral and coordination with primary health and behavioral health providers, financial management assistance, health and safety referrals, conducting regular and as needed apartment visits to ensure safety and security, prepare and maintain case records including individualized care plans and service notes. Also work in conjunction with the resident manager regarding tenant tent payment, eviction proceedings, management of front desk staff, and coordination of facility contractor activities. Provide emergency first aid/CPR and/or Narcan assistance when needed. Related duties as assigned.
Full-time, 37.5 hours per week
- Monday-Friday 8:00am-4:30pm
Qualifications
- BA in social work or related field required.
- Experience providing case management related services to mentally ill and/or substance abusing individuals required.
- Effective interpersonal communication and organizational skills, and staff supervision experience required.
- Knowledge of AWARDS and Yardi preferred. Bilingual in English and Spanish is a plus. CPR training certification or willingness to take training class in CPR.
