Infusion Suite Nurse - Per Diem - San Jose

Job Overview An Infusion Nurse II is a licensed registered nurse who coordinates and provides intermediate direct patient care to patients in the home or an alternate infusion suite, ensuring patient safety, continuity of care, and compliance with a physician’s plan of care. The role is performed under the structured supervision of the designated supervisor. Responsibilities Initiate, develop, and implement intermediate nursing plans of care, evaluating patient progress toward goals and modifying treatment plans in response to changing patient status or physician orders. Organize and participate in direct patient care, performing treatments, administering medications, and educating patients and families or caregivers. Assess patient needs and physical status at each skilled visit through health data access and patient interview, and re‑evaluate needs during subsequent visits. Obtain and clarify physician orders for plan of treatment revisions, promptly inform physicians of significant changes in a patient’s condition, and provide written summaries within supplemental orders. Provide training and mentorship to Infusion Nurse I’s, ensuring patient safety and compliance. Use a patient‑centered approach to teach patients and families to achieve independence with prescribed therapy and care needs through active participation. Effectively and timely communicate with Option Care’s clinical team, medical providers, patients, and families or caregivers to facilitate continuity of care. Prepare clinical documentation in real‑time during visits and submit it to the nursing department to comply with established billing timelines; complete all documentation legibly and apply approved abbreviations. Coordinate discharge planning and prepare discharge summaries with patient instructions, thoroughly reporting patient care needs, progress, and goals when transferring care. Demonstrate compliance with agency operations, Option Care’s policies and procedures, professional standards, local, state, federal regulations, and accreditation standards. Maximize work efficiency through the use of computers and other automation technologies to validate treatment orders, communicate patient care provided, and follow assigned visit schedules. Respond promptly and appropriately to patient requests, initiate emergency procedures as necessary, and accept accountability for own practice through ethical conduct. Follow established programs and practices within policies that reflect Option Care’s mission, values, and objectives. Observe legal and ethical guidelines for safeguarding patient confidentiality and proprietary information, adhering to HIPAA regulations. Speak knowledgeably about Option Care’s scope of services and effectively instruct patients regarding related financial obligations. Attend mandatory training modules, in‑services, and continuing education to maintain competency and licensure, demonstrating knowledge of patient care and management in the home or alternative settings. Provide oversight and coordination of paraprofessionals in the home setting, acting as supervisor in the absence of the designated supervisor or nurse manager as mandated by regulations. Participate in multidisciplinary team conferences and provide precepting, training, and mentoring for orientation, onboarding, and supervisory activities as assigned. Participate in nursing department on‑call responsibilities and perform other related duties as directed by the supervisor. If applicable: insert PICC/Midlines per physician order; troubleshoot or declog occluded central line catheters; monitor PICC/Midline and educate patients on infection prevention and routine care. Supervisory Responsibilities Position does not involve hiring, performance reviews, scheduling, or other supervisory duties such as promotions, pay increases, or discipline. Basic Education and Experience Active, unrestricted RN license in the state of practice. Minimum of 1 year of nursing experience. Current CPR certification required. Basic knowledge of computer operating systems and software applications. Demonstrated competency in patient care standards and infusion skill sets required for agency programs. Advanced certification and training applicable to infusion nursing preferred. Necessary skills for self‑evaluation for annual appraisal and professional growth planning. Must own a motor vehicle, have the ability to drive for business travel, and proof of liability insurance. Physical Demand Requirements Ability to lift up to 50 pounds (maximum 75 pounds) and exert up to 20 pounds of force occasionally. Requires walking, standing, kneeling, turning, pivoting, balancing, stooping, reaching overhead, grasping, pushing, pulling, lifting, and carrying. Fine motor skills and visual acuity for close and distant work, color vision, peripheral vision, and depth perception necessary. Potential exposure to blood, body fluids, hazardous substances, needle or sharps injury, chemicals, extreme temperatures, adverse weather, animals, and unpredictable home environments. Safety Requirements Closed‑toe shoes, facemask, goggles, gown and/or gloves, and functional communication device for routine and emergency situations. Travel Requirements Willingness to travel up to 100% of the time for business purposes. Preferred Qualifications & Interests Bachelor of Science in Nursing from an accredited school (preferred). 1–2 years of previous infusion nursing experience (preferred).
