APT | Accredited Physical Therapist

Join a dedicated healthcare team in the Midwest region as a Homecare Physical Therapist, where you will develop personalized care plans tailored to each patient's unique needs and goals. This role involves evaluating patients for home health or hospice services, collaborating closely with interdisciplinary teams, and supervising Physical Therapist Assistants (PTAs) to ensure optimal patient outcomes.

Key Responsibilities

• Conduct comprehensive initial assessments within required timeframes and create individualized, age-appropriate care plans focused on self-management and measurable goals.
• Maintain ongoing communication with physicians regarding patient progress and any emerging issues.
• Implement physician orders promptly and update care plans as necessary, involving patients and caregivers in the process.
• Manage time and resources efficiently to meet productivity targets while ensuring high-quality care.
• Supervise PTAs and homecare assistants, providing guidance and documenting their activities in patient records.
• Collaborate with multidisciplinary teams through meetings and direct communication to coordinate patient care.
• Assist patients in obtaining recommended equipment and initiate discharge planning from admission.
• Adhere to infection control protocols and maintain safety standards during all treatments.

Education

Bachelor's degree in Physical Therapy from an accredited program recognized by the American Physical Therapy Association and the American Medical Association is required.

Experience

• At least one year of experience in an acute care hospital setting.
• Minimum one year in a rehabilitation facility or equivalent, serving adult patients.
• Preferred: Six months of home care experience.
• Computer proficiency is a plus.

Certifications and Licenses

• Valid driver's license.
• American Heart Association Basic Life Support (BLS) certification.
• Current state license to practice physical therapy.

Physical and Mental Requirements

• Ability to stand and walk for extended periods and navigate stairs.
• Strong communication skills, both oral and written.
• Excellent interpersonal and organizational abilities.
• Capability to safely operate a vehicle for 2-4 hours daily.
• Comfortable maneuvering around home obstacles and tolerating environmental factors such as temperature changes, smoke, dust, and pets.

Working Conditions

Frequent exposure to communicable and infectious diseases, potential contact with biohazardous materials, and varying community environments. The role requires adaptability to different home settings and occasional encounters with hostile individuals.
