Job Summary
Physical Therapist, Outpatient. The Physical Therapist evaluates assigned patients and develops, implements, and modifies individualized treatment plans based on patient needs, goals, and physician direction. The role provides functional treatment programs, patient and family education, clinical documentation, and care coordination to support effective progression toward discharge and return to community activities.
Job Details
Required Qualifications: Degree in Physical Therapy from an accredited school; at least 1 year of outpatient clinical experience; Cardiopulmonary Resuscitation (CPR) certification.
Schedule: Day shifts, 8 hours per shift, 40 hours per week.
Responsibilities: Complete comprehensive patient evaluations; apply clinical reasoning to establish treatment plans and goals; provide discipline-specific therapy services; document care accurately and timely; ensure treatment is provided under valid physician orders and within applicable authorization requirements; support student learning when assigned; pursue continuing education and professional development; adhere to state practice laws, regulatory requirements, professional standards, safety procedures, and ethical codes of conduct.
Additional Information: The role includes developing functional treatment programs, educating patients, families, students, and coworkers, participating in quality improvement initiatives, maintaining professional licensure in good standing, and promoting patient and employee safety.
