HHA - Per Diem - 1st ShiftLocation: Jamestown, NY Schedule: Minimum of one weekend per 4-week scheduling period, with additional shifts as available Salary Range: Position starts at $18.65/hourJoin Our Team at Hultquist PlaceLutheran's Hultquist Place—an Assisted Living Program (ALP) operating in coordination with Licensed Home Care Services Agency (LHCSA) standards—is seeking a compassionate and dependable Certified Home Health Aide (HHA) to join our day shift working 6am-2pm.Our HHAs play a vital role in supporting residents' independence, dignity, and safety—especially during overnight hours when attentiveness and responsiveness matter most.Position Summary:Under the supervision of the Director of Patient Services (DPS) and licensed nursing staff, the HHA provides personal care and supportive services in accordance with each resident's individualized care plan and applicable ALP/LHCSA regulations. This role includes assisting with self-administration of medications (following facility training and protocols), overnight monitoring, and timely communication of any changes in resident condition.Qualifications:High School Diploma or equivalent requiredValid New York State HHA Certification required (or equivalent approved training/exam)OR Certified Nursing Assistants (CNAs) willing to complete HHA conversion testing are encouraged to apply Prior experience in assisted living, home care, or long-term care preferredMedication assistance experience preferred (training provided)Basic computer skills for EMR documentationKey Responsibilities:Resident Care & SupportProvide hands-on assistance with activities of daily living (ADLs), including bathing, dressing, toileting, grooming, and mobilityPromote resident independence, dignity, and choice in all interactionsConduct routine overnight rounds to ensure resident safety, comfort, and well-beingRespond promptly to call lights and resident needsMedication AssistanceAssist residents with self-administration of medications in accordance with ALP guidelines and facility policiesFollow all medication assistance protocols, including proper documentation and reportingImmediately report any medication concerns, refusals, or discrepancies to licensed nursing staffDo not interpret physician orders or independently administer medications outside of scopeMonitoring & ReportingObserve, document, and report changes in resident condition, behavior, or needs to the Charge Nurse/DPSAccurately complete documentation in the electronic medical record (EMR) in a timely mannerReport incidents, safety concerns, or unusual findings per policyCare Coordination & TeamworkCommunicate effectively with nursing staff, leadership, and interdisciplinary team membersSupport a safe and respectful environment for residents, families, and coworkersParticipate in shift-to-shift communication to ensure continuity of careEnvironment & Support ServicesProvide light housekeeping and resident support services as neededAssist with meal-related needs, hydration, and nourishment monitoring (as applicable overnight)Follow all procedures related to admissions, discharges, and transfersWhat We're Looking For:Compassionate, dependable, and attentive caregiversStrong observation and communication skillsAbility to work independently on overnight shifts while remaining part of a collaborative teamCommitment to resident-centered care and regulatory complianceWhy Join Lutheran?Supportive team environment and strong clinical leadershipOpportunity to expand skills (including medication assistance training and HHA certification pathway)Meaningful work making a difference in residents' lives every dayWe'd love to have you join our overnight care team—apply today!Lutheran Social Services is an equal opportunity employer that is committed to diversity and inclusion in the workplace.
