POSITION SUMMARY
The Enhanced Care Management (ECM) Program Manager is responsible for the leadership, oversight, implementation, and ongoing development of Harmonium’s Enhanced Care Management program serving individuals and families with complex medical, behavioral health, housing, and social needs. This is currently a part-time position; however, as our organization continues to grow, we anticipate the role will expand into a full-time management position. The ECM Program Manager supervises care coordination staff and oversees outreach, engagement, care planning, and service delivery to ensure high-quality, person-centered, trauma-informed, and culturally responsive services aligned with Wellness Collaborative requirements.
This position plays a critical role in developing and maintaining strong community partnerships with health plans, healthcare providers, schools, housing agencies, behavioral health organizations, community collaboratives, and social service partners to support coordinated systems of care and improve outcomes for participants. The ECM Program Manager supports staff through reflective supervision, coaching, training, and operational leadership while ensuring compliance with agency, county, state, and managed care requirements. The position may also maintain a limited caseload to support continuity of services, staff development, and program operations.
ESSENTIAL FUNCTIONS
RESPONSIBILITIES
Responsible for developing, maintaining and implementing program policies and procedures and ensure that program staff adhere to program and Agency policies and procedures.
Accountable for the recruitment of direct reports and participate in the interviewing process for potential staff, interns and/or volunteers.
Responsible for the development, implementation, and oversight of Enhanced Care Management services in alignment with Wellness Collaborative requirements.
Provide leadership and supervision to ECM staff, including Lead Care Managers and/or Housing Navigation staff.
Attend supervisor meetings and collaboration meetings with San Diego Wellness Collaborative.
Ensure quality, timely, and compliant completion of assessments, care plans, progress notes, referrals, and all required documentation.
Review and approve individualized care plans and related ECM documentation.
Support outreach and engagement efforts for high-need and hard-to-reach populations experiencing complex behavioral health, medical, housing, and social challenges.
Develop and maintain strong collaborative partnerships with community organizations, healthcare providers, school districts, housing agencies, behavioral health systems, managed care plans, and other stakeholders.
Represent Harmonium within community collaboratives, Health Plan meetings, county initiatives, and interdisciplinary care coordination meetings.
Assist in developing workflows, procedures, and training materials that support effective ECM service delivery and compliance.
Monitor program performance metrics, outcomes, and quality improvement initiatives to ensure achievement of program goals and contractual expectations.
Participate in care coordination activities and may carry a limited caseload as needed to support program operations and continuity of care.
Ensure services are delivered using trauma-informed, strength-based, culturally responsive, and person-centered approaches.
Support staff in identifying and connecting participants to community resources, including medical care, behavioral health services, housing supports, transportation, food access, educational services, and social supports.
Responsible for ensuring personnel issues are proactively addressed in accordance with agency policies, procedures, and timelines.
Attend required staff meetings, trainings, community meetings, and collaborative partnerships.
JOB COMPLEXITY
Work effectively across multiple systems including healthcare, behavioral health, education, housing, and social services to support coordinated participant care.
Under HR’s direction will participate in preparing and reviewing in any legal related matters.
Responsible for developing trainings in collaboration with the Learning & Development Committee to ensure trainings reflect agency core values and initiatives.
Implement and maintain policies and procedures utilizing Trauma Informed Care Standards.
Support compliance with agency, Wellness Collaborative, and Health Plan requirements related to ECM services and documentation.
Provide crisis support, de-escalation assistance, and problem-solving support to staff and participants as appropriate.
Develop and implement strategies to improve participant engagement, outreach effectiveness, and care coordination outcomes.
Collaborate with leadership to identify service gaps, operational challenges, and opportunities for program growth and improvement.
Utilize data systems and reporting tools to monitor program performance, service delivery, and compliance metrics.
Support staff in addressing barriers impacting participant stability and wellness, including housing instability, behavioral health concerns, transportation barriers, and access to care.
Ensure services are coordinated and integrated across providers and systems of care.
Maintain awareness of community resources and establish collaborative relationships to strengthen referral networks and participant support systems.
STAFF SUPERVISION
Provide assistance and guidance on how to troubleshoot program and staff performance challenges.
Supervise, coach, and mentor program staff, interns, and volunteers while promoting professional growth and accountability.
Utilize reflective supervision and strength-based leadership practices to support staff development and retention.
Delegate responsibilities appropriately while maintaining accountability for program outcomes and service quality.
Provide guidance and support in addressing participant needs, care coordination challenges, and documentation requirements.
Support staff in developing effective engagement strategies with participants and community partners.
Conduct performance evaluations twice annually with each assigned supervisee.
COMMUNICATION
Develop and maintain positive, professional relationships with San Diego Wellness Collaborative personnel, community collaboratives, school personnel, parents, children and staff.
Responsible for implementing a quality professional development training program, in collaboration with HR to ensure consistency with the agency priorities and goals.
Provide regular, formal, thorough, accurate and timely updates on projects, program operations, issues and emerging risks and document as appropriate.
Serve as a liaison between Harmonium and external partners to support coordinated care and effective service delivery.
Facilitate positive communication through trauma-informed, strength-based, and solution-focused approaches.
Prepare and deliver professional presentations and reports tailored to management, staff, funders, and community stakeholders.
Participate in interdisciplinary meetings and collaborative planning efforts to strengthen systems of care and community partnerships.
AGENCY LEADERSHIP
Responsible for the overall leadership for all aspects of program operations including review, evaluation and strategic planning.
Provide leadership in the program’s achievement of goals and outcomes using measurement and indicator tools to demonstrate progress and success.
Inspire the team in applying solution-based approaches, generate new ideas, and expand program within Harmonium’s mission, vision, initiatives and priorities.
Represent Harmonium in a positive, professional manner and lead by example.
Create and support a high performing, team-based culture aligned with values to motivate and inspire staff to work collaboratively toward mission and vision.
Participate as a member of Program Leadership Team (PLT) including Ad Hoc Committees.
Actively participate in planning and attending Harmonium events.
Oversee and manage data system utilized by the program.
Serve as a subject matter expert regarding ECM services, care coordination practices, and community-based service delivery.
Foster a high-performing, collaborative team culture grounded in equity, inclusion, accountability, and relationship-based practices.
DECISION MAKING
Participate in the decision-making process regarding fiscal and programmatic matters.
Apply Harmonium and program policies, guidelines, SDUSD policy and Title 22 Regulations when making decisions.
MINIMUM COMPETENCIES
EDUCATION & EXPERIENCE
Must hold a current and active California Clinical license (LCSW, LMFT, LPCC) under the CA Department of ConsumerAffairs Board of Behavioral Sciences for a minimum of two years
Masters Degree required in social work, public health, counseling, education, or a related field.
A minimum three years of direct supervision experience preferably working with children and families.
Experience working within Enhanced Care Management (ECM), care coordination, case management, behavioral health, healthcare, housing navigation, or community-based services strongly preferred.
Experience working with Medi-Cal, managed care populations, CalAIM initiatives, and/or high-acuity populations preferred.
Experience with the reflective practice process.
Experience with housing and homeless services preferred.
Experience developing community partnerships and collaborative systems of care strongly preferred.
Experience utilizing trauma-informed, culturally responsive, and person-centered practices.
KNOWLEDGE, SKILLS & ABILITY
Knowledge of Enhanced Care Management (ECM), CalAIM, Medi-Cal Managed Care, and care coordination best practices.
Strong understanding of behavioral health, healthcare systems, housing resources, and community-based support services.
Ability to develop collaborative relationships across diverse systems and stakeholders.
Ability to communicate effectively in written and verbal form using positive interpersonal skills.
Strong leadership, organizational, and problem-solving skills.
Ability to manage multiple priorities in a fast-paced environment.
Ability to utilize electronic health records, data tracking systems, and reporting tools effectively.
Demonstrated ability to work independently and collaboratively within multidisciplinary teams.

ADDITIONAL EMPLOYMENT REQUIREMENTS
Must be 18 years of age or older
Must receive Fingerprint Clearance upon hire (paid by Harmonium)
Valid TB results required
Valid CA driver’s license and access to a reliable vehicle with active insurance
