Job Requirements

• Active New York LPN license
• Current BLS certification
• At least 1 year of LPN experience in a long-term care or skilled nursing setting
• Experience caring for adult and geriatric residents
• Ability to work collaboratively with RNs, physicians, and interdisciplinary healthcare team members
• Strong communication, organization, and critical thinking skills
• Ability to maintain professionalism, confidentiality, and a resident-centered approach to care

Responsibilities

• Provide direct and indirect skilled nursing care to assigned residents in a long-term care setting
• Collect and report resident assessment data to the RN and interdisciplinary care team
• Support the development and implementation of individualized plans of care
• Monitor resident conditions and recommend updates to the plan of care as needed
• Promote a safe, supportive, and respectful environment for residents and families
• Collaborate with nursing staff and other healthcare professionals to support positive resident outcomes
• Maintain current nursing knowledge through continuing education and in-service learning
• Participate in mentoring and orientation activities as directed
