What You'll Do Travel to patients' homes, assisted living communities, hospitals, and skilled nursing facilities to complete hospice admissions. Perform comprehensive clinical eligibility assessments and initiate hospice services. Lead compassionate conversations about goals of care, hospice philosophy, Medicare benefits, and next steps . Build trust with patients, families, physicians, and referral partners from the very first interaction. Coordinate seamless transitions into hospice care with the interdisciplinary team . Complete timely, accurate EMR documentation while maintaining regulatory compliance. What You'll Bring Active Minnesota Registered Nurse (RN) license. Experience in hospice preferred , or a strong background in oncology, ICU, palliative care, home health, emergency medicine, or another high-acuity clinical setting. Outstanding communication, assessment, and relationship-building skills . Confidence leading sensitive conversations with patients and families. Ability to work independently in a field-based role .
