Role Overview The Enhanced Care Management (ECM) Care Manager supports individuals and families in addressing their health and care management needs. The Care Manager uses independent judgment and discretion to provide direct services including case planning, advocacy, outreach, and home visitation activities for children and families in collaboration with a team. Responsibilities Outreach to potential members from a call list (MIF), inform them of the program and its benefits, and enroll them in services. Outreach includes phone calls, in-person visits, letters, emails, and text messages. Complete CII's intake forms and a comprehensive assessment, care plan, and Health Risk Assessment (HRA). Ensure goals of individuals from care plan are being met. Serve as a liaison between member, healthcare team, and other community resources. Link family to relevant and requested community resources. Assist member and their support system with health literacy. Provide health promotion and self‑management training. Advocate on behalf of member with healthcare professionals and community programs. Identify community resources for member via internet research, community directories, and/or staff assistance. Accompany ECM members to office visits, as needed and according to applicable guidelines. Monitor treatment adherence and inform teams of any changes or lack of adherence. Arrange transportation as needed. Ensure all client files are accurately and completely maintained. Establish and maintain rapport and effective working relationships with children and families and work effectively within CII and the social work community at large. Use independent judgment and discretion to identify client’s needs and provide community referrals to client and others. Advocate on behalf of the client with other agencies, government programs and similar to help them receive needed services. Ensure accurate and timely completion of outcome and evaluation testing and reports required by the program. Travel extensively within LA County and on occasion beyond LA County. Additional Duties Conduct intake interviews to gather information and complete required paperwork. Attend educational workshops reviewing professional publications, establishing personal networks, benchmarking state‑of‑the‑art practices, and participating in in‑service programs. Develop and maintain knowledge of service standards and policies as stipulated by contract, licensing, and/or other governing bodies. Provide transportation to clients based on needs to various appointments. Other duties and special projects as assigned. Supervisory Responsibilities This position has no supervisory responsibilities. Qualifications Education & Experience 2+ years experience providing case management in an intensive level program. Bachelor's degree in Social Work, Health, Public Health, or a related field with relevant work experience. Background in health care services and/or public health. Ability to engage and connect with diverse populations, including but not limited to individuals experiencing homelessness, at‑risk for homelessness, and under‑resourced communities, persons with mental health and substance abuse disabilities. Comfortable with conducting outreach virtually, in‑person, and by phone, including making cold calls. Detail‑oriented – can complete the necessary paperwork (intake forms, Health Action Plan, etc.). Available to work evenings or weekends, as needed. Other Qualifications Knowledge of and strong engagement skills, including crisis and de‑escalation skills to provide quality care and services to clients and families. Reliable transportation; possesses a valid driver's license and state‑required auto insurance. Spanish/English bilingual preferred. Ability to handle confidential information. Strong interpersonal skills and critical thinking abilities. Ability to work independently and take initiative. Establish and maintain effective working relationships with those contacted in the performance of required duties. Maintains patient privacy, including protecting the confidentiality, safeguarding the integrity and availability, and limiting the use and disclosure of patient Protected Health Information (PHI) to authorized individuals and entities. Participates in, conducts, and sustains performance quality improvement activities according to designated role and responsibilities. Sensitivity to service population's cultural and socioeconomic characteristics. Excellent verbal and written communication skills. Punctual and dependable attendance. Certificates, Licenses, Registrations None required. Computer Skills Intermediate knowledge of computer programs in a Windows environment. Database software proficiency. Proficient in Electronic Health Records System. Vision Requirements No special vision requirements. Noise Level Moderate noise. Training Requirements New Hire Orientation. Mandated Reporter training. WRAP program only – Wraparound basic training. Family Preservation program only – Family Preservation basic training. AB-1343: Sexual Harassment Prevention training. Compensation $26.75 USD - $32.10 USD. In accordance with California law, the expected pay range for this position is included in this posting. The actual compensation will be determined based on factors permitted by law. New hires will be offered a salary at the starting point of the range to maintain internal equity and allow opportunity for future salary growth. CII salaries are determined based on comprehensive internal and external market analysis, as well as the skills, education, and experience relevant to each position.
