A nationally recognized value-based healthcare organization is seeking a Primary Care Physician for an established outpatient center in Cleveland. The practice serves primarily Medicare-eligible adults and provides physicians with smaller patient panels, extensive care-team support, and a schedule designed for long-term sustainability.

PRACTICE HIGHLIGHTS

• Monday Friday daytime schedule
• No nights or weekends
• Outpatient responsibilities only
• Approximately patients per day
• Smaller patient panels

PHYSICIAN RESPONSIBILITIES

• Provide comprehensive primary care to adult and geriatric patients
• Manage acute and complex chronic medical conditions
• Develop individualized preventive-care and treatment plans
• Coordinate specialty, behavioral health, hospital, and community services
• Lead an interdisciplinary care team
• Build meaningful, long-term relationships with patients and families
• Maintain accurate and timely clinical documentation

QUALIFICATIONS

• MD or DO
• Board Certified in Family Medicine, Internal Medicine, or Med-Peds
• Recently trained Board Eligible physicians may be considered
• Current Ohio medical license or ability to obtain one
• DEA registration or ability to obtain it
• Comfortable caring for medically complex adults and seniors
• Interest in value-based care, population health, and team-based medicine
