LPN/LVN – Long-Term Care
Location: Utica, NY 13502
Duration: 13 weeks with a high possibility of extension
Shift: 10:30 PM–6:30 AM
Schedule: 37.5 hours/week
Break: 30-minute unpaid break
Position Overview
We are seeking an experienced Licensed Practical Nurse (LPN/LVN) to provide nursing care in a long-term care environment. The ideal candidate will have previous LTC experience, travel nursing experience, and proficiency with PointClickCare EMR.
Key Responsibilities- Provide nursing care to residents in a Long-Term Care (LTC) setting.
- Administer medications and treatments according to established orders and protocols.
- Monitor residents' conditions and report changes to the appropriate healthcare team.
- Maintain accurate and timely nursing documentation.
- Communicate effectively with residents, families, physicians, and other members of the healthcare team.
- Follow facility policies, procedures, safety standards, and infection-control practices.
- Utilize PointClickCare EMR for patient documentation and clinical records.
- Promote resident safety, comfort, dignity, and quality of care.
- Collaborate with nursing staff and other healthcare professionals to support resident needs.
- Minimum 1 year of experience in a Long-Term Care (LTC) facility .
- Previous travel nursing experience with supporting documentation required.
- Active New York State LPN license required.
- BLS-AHA certification required.
- High School Diploma required.
- Experience with PointClickCare EMR required.
- Strong communication, clinical assessment, and organizational skills.
- Ability to work independently and effectively as part of a multidisciplinary healthcare team.
- Night Shift: 10:30 PM–6:30 AM
- 37.5 hours/week
- 30-minute unpaid break
- Contract: 13 weeks with a strong possibility of extension
