Social Services Coordinator

Springfield Suites Rehab & Nursing Center is seeking a full-time Social Services Coordinator to join our skilled nursing team. In this role, you will oversee social services, admissions support, and discharge planning while serving as a key resource for residents and their families throughout their stay. Salary: $45,000 per year Responsibilities Complete social services assessments, including BIMS, PHQ-9, and trauma screenings Review admission documentation for appropriateness of placement Coordinate discharge planning and aftercare services Arrange home care, medical equipment, follow-up appointments, and community resources Communicate with residents and families regarding concerns and care needs Coordinate care plan meetings Maintain advanced directives and resident records Document assessments and services in PointClickCare Qualifications Experience in social work, admissions, and/or discharge planning in a nursing home or long-term care setting required Knowledge of the Medicaid process required Degree in Social Services, Human Services, Social Work, or a related field preferred Strong communication, organization, and problem-solving skills Benefits Medical Insurance Dental Insurance Vision Insurance 401(k) Paid Time Off Supportive team environment Join our team and make a meaningful difference in the lives of residents and families every day. #J-18808-Ljbffr
