As a Licensed Vocational Nurse Care Coordinator, you will work onsite and play a crucial role in facilitating communication between patients, caregivers, physicians, and the outpatient care team to optimize health and wellness. This position is perfect for someone who is passionate about reducing inpatient admissions and improving medical stability in patients with complex comorbid conditions.

In this role, you will be responsible for:

• Managing a caseload of 10,000-15,000 patients with identified gaps in care in collaboration with the Health Care Team.
• Collaborating with the PCMH Team in the design, implementation, and evaluation of the PMH model as applicable.
• Coordinating preventative and follow-up care for all patients receiving care from participating providers.
• Reviewing patient medical records to identify care gaps and scheduling needed preventative services using evidence-based protocols.
• Fostering a team approach by working collaboratively with members, families, PCPs, and other healthcare team members to ensure coordination of services.
• Assisting Nurse Navigators in performing transition of care outreach to discharged patients to schedule return visits with the PCP per guidelines.
• Employing knowledge of evidence-based guidelines to effectively communicate the importance of recommended care.
• Must be licensed in Texas or have a compact license.
