OSF ITEMS OF NOTE:
- Local Rate: Applies to clinicians with permanent addresses within 50 miles (-*** to regular and OT bill rates)
- OSF Call Off Policy: Equivalent of 12 hours every 13 weeks with no penalty or need to pay for the shift
- Non-billable Orientation Hours: 12 hours, guaranteed hours will not apply during orientation


REQUIREMENTS FOR SUBMISSION:
- Last four (4) digits of clinician's SSN
- Clinician's middle initial
- Requested time off
- Your candidate understands OSF is a Drug-Free Campus and they must produce a negative drug screen for all panels

Unit OR - Surgery - General (Major) - RN
# Suites 5 and 2 procedure rooms
Average # of cases per day 20-30
Required Certifications (i.e., BLS, ACLS?) BLS, ACLS preferred but not required
Common Cases General, ortho, gyne, podiatry, endoscopy, opthamology
Experience or 'Must have' skills EPIC
Scrubbing required - RN's Not required
Is there a Charge Nurse &/or House Supervisor on each shift? Does the charge nurse take an assignment? There is a supervisor present during the day shift who does not normally take an assignment. We only run one shift with call. When call begins there is a house supervisor available.

Shifts & Scheduling (Do you allow self-scheduling? Block scheduling?) We do allow self scheduling
Weeknight/Call Requirements 3:15pm-6:45am
Weekend/Call Requirements Sat 7am-Monday 7am (48 hours)

What is call response time? 30 minutes
Floating Requirements none
Scrub Color/Dress Code hospital provided
Unit/Department specific orientation in unit orientation as needed
