In-Home Nurse Practitioner or Physician Assistant

Signify Health is seeking an in-home Nurse Practitioner or Physician Assistant to perform clinical evaluations for health plan members in their homes. You will complete comprehensive assessments, review medical histories, reconcile medications, and identify care gaps using Signify’s technology platform. Collaborating with a multidisciplinary team, you’ll document findings, educate patients, and support value-based care plans that improve outcomes for vulnerable populations while enjoying flexible, part-time work and a supportive, mission-driven culture.
