License: BSN required

Certifications: Required: BSN; Preferred: CCM or related certification

Must-Have: Extensive background and knowledge of current methods for assessing physical and mental impairment. Understanding the characteristics of illness and expected treatment. Knowledge of benefits, plan contracts, eligibility issues, and requirements.

Description: The Care Manager will utilize a collaborative process to assess, plan, implement, monitor, and evaluate the options and services required to meet a patient's health needs. The Care Manager will use effective communication and available resources to promote quality, cost-effective outcomes for 18-22 patients. The role involves managing length of stay and promoting efficient utilization of resources. The Care Manager will determine medical necessity and appropriateness of admission using screening criteria.
