Your Role You will oversee the clinical care of an assigned caseload, ensuring optimal outcomes, compliance with regulatory standards, and strong partnerships with clients, families, and healthcare professionals. You will assess client needs, develop personalized care plans, and support caregivers in the field with expert guidance and clinical leadership. What You’ll Do Conduct client assessments, develop care plans, and adjust as needs evolve Provide clinical supervision and direction to caregivers and nurses Collaborate with families to set goals and align daily care with those priorities Maintain accurate, timely, and compliant client documentation in the EMR and home records, ensure prompt communication of status changes, and oversee information sharing with families, practitioners, and stakeholders to support continuity of care and regulatory compliance.
