If you're excited to shape the future of home-based care, here's what the role involves.Position DescriptionThe Clinical Manager, in collaboration with the Clinical Supervisor, is responsible for the provision of high-quality patient care for home health clients in branch(s) assigned.Essential Duties and ResponsibilitiesRegistered Nurse (RN) with 3+ years of home health field experience preferredAt least 3 years of clinical management experience is preferred but not requiredStrong understanding of OASIS documentation, home care protocols, and Medicare complianceExperience with Homecare Homebase (HCHB) preferredAvailable during operating hours, which includes all hours in which the HHA is open and provides patient care.Demonstrates and advocates accepted standards of practice, including the use of standard precautions to prevent the transmission of infections and communicable diseases.Provides feedback to all clinical staff about their performanceResponsible for completion of in-home annual and as needed competency and evaluationsManages the clinical care teamsIf designated, performs the role of an alternate administrator in their absenceAble to lift up to 40 lbs.; role involves walking, standing, sitting, bending, and hand useExcellent auditory, verbal, and written communication skillsProvide clinical leadership and support to field staff to ensure high-quality, compliant careConduct chart audits, review visit notes for accuracy, and ensure Medicare criteria are metReview and submit OASIS to the state, correct errors, validate reports, and meet RAP and Final Claim deadlines (NOA: within 5 days; Final Claims: within 10 days)Monitor OBQI/OBQM indicators and implement corrective actions as neededManage clinical care teams and productivity expectationsComplete in-home annual competencies, evaluations, and supervise case management communicationDeliver feedback, conduct staff development training, and assign preceptors for new clinical staffPromote team members' professional growth and serve on Performance Improvement and other committeesPerform concurrent audits of at least 10% of the active censusCollect, analyze, and report clinical data to the Administrator; develop and monitor Plans of CorrectionFacilitate team case conferences and participate in management and staff meetingsServe as backup for on-call duties when the on-call nurse is unavailableProvide intermittent skilled nursing visits, including SOCs and ROCs, as neededSupport patient care delivery in the field when staffing or acuity requiresEnsure alignment with agency policies, procedures, and professional standardsAssist the Administrator with staff orientation and maintain communication with physicians, referral sources, and the broader care teamCarry out additional duties as assigned by the AdministratorRequired Knowledge, Skills and ExperienceCurrent RN license in the state of practice, or eligibility per Nurse Licensure Compact (NLC) guidelinesActive CPR certification requiredMust maintain a valid driver's license and a clean driving recordDemonstrated leadership abilities and strong knowledge of home care clinical practices, Medicare regulations, and federal, state, and local compliance standardsProven ability to effectively manage agency resourcesSkilled in assessing patient needs and developing individualized care plansProficient in personal computer use, including email, EMRs, word processing, spreadsheets, and presentation toolsAble to lift up to 40 lbs. and perform duties that require walking, standing, sitting, bending, and use of handsExcellent auditory, verbal, and written communication skillsPerks and Benefits (What we offer):Competitive salary and multiple health benefit optionsCareer growth and developmentMentorship and continued learning opportunitiesEngaging and mission driven workplace401(k) matchingMileage reimbursement (if eligible)Every person on our team helps shape the future of Alpine Care Home Health.
