About the Role

As a Certified Home Health Aide, you will play a vital role in delivering high-quality care to patients in the comfort of their own homes. You will work as part of a multidisciplinary team to provide personalized care to patients who require assistance with daily living activities.

Responsibilities

• Provide assistance with daily living activities such as bathing, dressing, grooming, and ambulation

• Assist with medication management and wound care

• Monitor vital signs and report changes to the healthcare team

• Maintain accurate records of patient care and services provided

• Collaborate with patients, families, and healthcare professionals to ensure comprehensive care

Requirements

• High School diploma or equivalent required

• 1 year of experience as a Certified Home Health Aide or related field

• Current certification as a Certified Home Health Aide (CHHA) or willingness to obtain

• Ability to work evenings, including weekends and holidays as needed

• Strong communication and interpersonal skills

• Compassionate and patient-centric approach to care
