Case Manager

Cardinal Hill Rehabilitation Hospital, part of Encompass Health, seeks a Case Manager to coordinate care for patients in our inpatient rehab facility. You’ll complete assessments, develop individualized discharge plans, and lead communication between patients, families, therapists, nurses, and physicians to support recovery after stroke, brain injury, spinal cord injury, and other complex conditions.
