Responsibilities
- Evaluate referred patients and develop treatment plans to improve independence in activities of daily living and functional tasks
- Implement OT interventions and therapeutic activities to support recovery, safety, and functional independence
- Educate patients and families on home programs, activity modification, and safety to support discharge planning
- Document evaluations, goals, plan of care, progress notes, and discharge planning; communicate with physicians and the care team
- Collaborate with an interdisciplinary team to optimize outcomes and coordinate care, including support for transition to the next level of care
- Maintain licensure, CPR certification, and safety standards; ensure accurate charting and compliance with facility policies

Requirements
- Oregon Physical Therapist licensure or equivalent as required by the Oregon State Board of Health
- Current CPR certification
- Education: Bachelor of Science or Master’s degree in Physical Therapy from an APTA-accredited program
- Current valid Oregon driver’s license
- Experience: Desired, but not required
- Ability to work in a rural, acute care setting
