Your Future Role At A GlanceLocation: Tulsa, OKFacility: Ascension St. John's Medical CenterDepartment: Case ManagementSchedule: Full-Time, Monday-Friday, 8am-4:30pmHow You'll Make An Impact In This RoleMentor Colleagues: Help junior case managers with complex cases and prioritizing their tasksImprove Processes: Review case management data to identify trends and enhance workflowsPrevent Readmissions: Develop detailed discharge plans to lower 30-day readmission ratesEnsure Continuity of Care: Support patients from admission through discharge to prevent care gapsWhat Minimum Requirements You'll NeedLicensure / Certification / Registration:Licensed Registered Nurse credentialed from the Oklahoma Board of Nursing obtained prior to hire date or job transfer date required.Case Manager credentialed from the American Case Management Association (ACMA) preferred.Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.Education:High school diploma/GED with 2 years of experience, OR Associate's degree, OR Technical degree required.
