Ascendant Health has a team of skilled nurse case managers to navigate the healthcare system, prevent medical issues, provide choices, tailor treatments, and manage care complexities. Role Description This is a part-time hybrid Nurse Case Manager role located in the Traverse City area, with some flexibility for remote work. The Nurse Case Manager will coordinate all aspects of injury-related care, advocate for their patients, and develop a care plan to manage care complexities.
