Field-Based Community Health Advocate
Molina Healthcare
Molina Healthcare seeks a Community Health Worker to support community-based member advocacy and assist vulnerable members in navigating health care needs. You will engage with members in homes, shelters, and provider offices, empowering them to maximize benefits and access social services. Travel will be 50-80% locally to meet members, providers, and community resources, contributing to strategy for quality, cost-effective care.
