What You’ll Do

Client Assessments & Care Plans – Evaluate needs and design personalized care plans
Coordinate Services – Partner with nurses, caregivers, and providers for seamless support
Advocate for Clients – Be a trusted voice for patients and families
Maintain Compliance – Keep records accurate and services aligned with state and federal guidelines
Crisis Support – Step in quickly when urgent issues arise
Monitor & Adjust – Track progress and adapt care plans to improve outcomes

What We’re Looking For

Excellent communication & relationship-building skills
Strong organization and time-management abilities
Knowledge of Medicaid, Medicare, and Indiana home care guidelines
Background in case management, social work, or healthcare
Tech-savvy with EHR or case management systems
Bilingual (English/Burmese) is a plus—immediate need!
