Responsibilities
- Coordinate inpatient care for adult patients, including assessment, planning, and ongoing management of discharge needs.
- Develop and implement individualized care plans in collaboration with the multidisciplinary team, balancing patient goals with appropriate utilization of services.
- Facilitate safe and timely transitions to post-acute care, home health, or community resources; provide patient and family education as needed.
- Document care coordination activities in the electronic medical record and maintain clear communication with physicians, nurses, social workers, and other team members.
- Monitor compliance with regulatory and payer guidelines, contributing to appropriate utilization reviews and length-of-stay management as applicable.

Requirements
- Active Registered Nurse license in California
- CPR or BLS certification from an AH-accepted vendor (required)
- Valid Driver’s License, 21 years of age or older, with current auto insurance
- Ability to work in a hospital-based case management environment
- Experience in hospital-based case management or discharge planning is preferred
- Education: RN to practice; degree level as required by California licensure
