Job Details Job Location: Duncan, OK 73533 Position Type: Full Time Salary $36,000 Responsibilities Complete individual, family, and group outreach to clients as assigned to include the following: Teaching skills including but not limited to the following: coping skills, decision-making skills, anger management skills, life skills, conflict resolution skills, communication skills, and independent living skills. Provide case management services, information, education, and referral services. Provide services to CARS clients as assigned, completing the following: An initial referral conference at intake including the client, family, and OJA worker A monthly report for each CARS client Maintain contact with OJA staff in regard to each client referred Be on call to clients through either an agency cell phone or a second number downloaded on your personal phone Provide mentoring services to clients as assigned which include but are not limited to: Completing initial assessment and follow up assessments every three months Providing assistance to youth with educational problems and deficiencies Advocating on behalf of the youth with school personnel, parents, community service sites, etc. Teaching life skills. Assisting age appropriate youth in seeking and obtaining employment. Encouraging youth to attend counseling appointments, if applicable, and maintaining communication with the client’s counselor and other members of the treatment team. Assisting youth in participating in appropriate leisure activities to improve social skills and increase positive peer interaction. Acting as a role model for youth while engaging them in community activities. Meeting with OJA Unit staff to ensure effective communication for those youth on probation, if applicable. Assist youth in completing community service necessary for completion of probation, if applicable. Provide weekly services to each client on caseload. Make contact with each new client within five days of the referral and document this in Chart Caddy. Complete assessments on each client, including thorough completion of each intake document and assessment tool. Complete treatment plan development, set goals and objectives for each client in the client’s own words, according to established time frames, assuring that all appropriate information is included and shared with treatment team members. Assist with ensuring that each client and/or parent/guardian is offered a copy of the treatment plan goals and objectives. Complete case management/individual outreach and discharge planning components of the client’s treatment plan and reviews, documenting discussion of the plan with the client, case worker, or family as appropriate. Complete progress notes in Chart Caddy after each session scheduled, including cancelations, no shows, and completed sessions. Also, complete information only notes in Chart Caddy for each client who was not able to be scheduled that week. Turn in weekly billing by the assigned deadline each week. Complete progress notes and turn in verifications by the assigned deadline each week. Complete treatment plan reviews at six-month intervals, including completion of necessary assessments which were initially completed at intake. Develop appropriate aftercare plans with the client, assessing the need, and arranging for community support as appropriate and documenting these plans. Contact each client after a no show to determine if the client is still interested in services and remind the client of the no show policy. Complete corrections on all case files, progress notes, and treatment plans as provided by the clinical director within a timely manner. Complete prevention courses as assigned which could include Parenting, Anger Management, Botvin Life Skills, or First Time Offender Classes. Duties include: Completing a sign in sheet for each class completed Complete pre and post tests and surveys for all courses Teach curriculum provided for each course Maintain communication with clinical director by reading and responding to emails daily. Participate in supervision at least twice per month with clinical director. Maintain case management certification as applicable and regularly update clinical skills. Work a flexible 40-hour week Monday through Sunday. Weekend work may be required in cases of emergency intervention or occasional weekend classes. Abide by professional ethical guidelines set forth by Marie Detty Youth and Family Services, the Office of Juvenile Affairs, ODMHSAS, and applicable federal guidelines. Refrain from documenting any inaccurate dates and/or times that services were provided. Always record what was actually provided on any and all documentation. Provide community development and/or education to the community as assigned by the clinical director. Responsible for completing the required minimum number of training hours per year (24 hours/full-time; 12 hours/part-time). Other duties as required by program needs. Competencies Demonstrates ability to provide psychoeducational curriculum in a variety of settings including individual, family, and group settings. Demonstrates ability to teach CBT skills to youth and adults. Completes all surveys as required for each type of client served. Demonstrates thorough knowledge of community resources to provide to and effectively advocate for clients. Maintains an average of 20-25 hours of billable services weekly. Maintains a 40-hour work week on a consistent basis and demonstrates dependability by consistently following proper procedures when unable to work as scheduled. Completes treatment plans and progress notes in a timely manner that are thorough and complete. Completes or participates in completion of discharge plan for clients who have left the program within 2 weeks of discharge date. Demonstrates extensive knowledge of community resources and has built relationships with school personnel and other community members. Demonstrates ability to work effectively with clients to achieve their goals. Works as part of the team to provide comprehensive care to clients. Understands and applies the agency policies and procedures (i.e. dress code and appearance, adherence to work schedules, completion of trainings, etc.). Punctually attends meetings and trainings. Completes assigned trainings in adequate time frames and obtains the minimum number of training hours required within the calendar year. Demonstrates dependability by consistently working scheduled shifts and following proper procedures when unable to work scheduled shifts. Displays cultural competency by valuing the fundamental rights, dignity, and self-worth of all people including respecting age, culture, disability, ethnicity, race, religion/spirituality, gender, sexual orientation, marital status, socioeconomic status, or any basis proscribed by law. Interacts with others in a professional manner and effectively navigate interpersonal differences. Maintains appropriate boundaries with supervisors, colleagues, and clients. Demonstrates the ability to consult or seek supervision regarding concerns or issues and communicates effectively with colleagues and clients. Identifies potential safety issues and takes measures to reduce risks and protect clients, staff, and themselves. The Outreach Specialist will receive training and be evaluated based on each of the above competencies and responsibilities. This is a full time 40-hour per week salary position classified as exempt from overtime according to the provisions of the Fair Labor Standards Act. Marie Detty Youth and Family Services is an at-will employer. Employment is by mutual consent of the company and employee and either may terminate the relationship at any time, with or without cause or advance notice. This job is contingent upon funding; however, availability of funding is not a guarantee of continued employment, nor does it imply any specific terms of employment. QualificationsRequirements Must be 21 years old. Must have a minimum of a bachelor’s degree in a behavioral health field of study, or a bachelor’s degree in a related field of study with adequate training and experience to include no less than two years of experience working with youth and families. Must come a Case Manager II within 60 days of hire. Demonstrate strong writing, verbal, and organizational skills. Must have strong assessment and interpersonal skills, flexible attitude, and able to meet the demands of a heavy caseload. Must have a valid driver’s license and telephone. Must pass FBI background investigation. Demonstrates effective communication skills and ability to network with others. Ability to communicate with and advocate on behalf of clients. Ability to maintain records and adhere to confidentiality. Ability to maintain a professional appearance and demeanor at all times. Adhere to all regulations concerning confidentiality, judgment, tact, and diplomacy. Ability to assess situations, identify potential safety issues, and independently initiate safe interventions. Must be able to attend mandatory trainings and monthly staff meetings as scheduled.
