PRN Physical Therapist (PT) – Home Health Diamond Home Health Care Inc. | Grand Rapids, MI Flexible Schedule | Per Visit Pay | No Holidays or Weekends EverHome is seeking a compassionate and experienced PRN Physical Therapist to join our team, providing in-home therapy services in the Grosse Pointe area. If you’re looking for flexible, meaningful work without the pressure of minimum hours or weekend coverage, we’d love to connect. Position Highlights PRN, flexible scheduling – pick up visits based on your availability No weekend or holiday requirements Per visit pay – rate varies by visit type (evaluation, follow-up, discharge, etc.) Coverage area: Grosse Pointe Electronic documentation via WellSky (formerly Kinnser) What You’ll Need Active Michigan PT license At least 1 year of home health experience required Strong time management, communication, and documentation skills Reliable transportation and ability to travel within assigned area Benefits We are a dedicated, clinician-first agency that prioritizes quality care, autonomy, and flexibility.
