Title: RN – Adult Care / Home Care Nurse

Location: Bronx Borough

State: New York

Organization: NYC Health + Hospitals

Duration: 13 Weeks (High possibility of extension or permanent placement)

Schedule: Day Shift | 5x8-Hour Shifts
Weekly Hours: 40 Hours
Weekend Requirement: Every Other Weekend Required

Position Overview:

"Navitas Healthcare, LLC" is seeking an experienced RN – Adult Care / Home Care Nurse to provide high-quality nursing services to patients in their homes throughout the Bronx Borough. The RN will deliver comprehensive patient-centered care, conduct clinical assessments, administer treatments, and collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimal patient outcomes.

Primary Responsibilities:

• Assess, plan, implement, and evaluate individualized patient care plans

• Provide direct nursing care to adult home care patients

• Administer prescribed medications and treatments

• Perform wound care, IV therapy, catheter insertion, and post-operative care

• Monitor patient vitals and report adverse reactions or changes in condition

• Educate patients and families regarding treatment plans and home care instructions

• Maintain accurate documentation, patient files, and health records

• Collaborate with healthcare providers on discharge planning and continuity of care

• Ensure adherence to nursing standards, infection control, and clinical protocols

• Utilize OASIS documentation and EPIC EMR systems as required

Experience & Requirements:

• Minimum 2 years of recent Med/Surg experience required

• Home care nursing experience required

• OASIS knowledge required

• EPIC EMR experience preferred

• Must be comfortable traveling to patient homes within Bronx Borough

Education Requirements:

• Associate Degree in Nursing (ADN/ASN) from an accredited institution required

Licensure / Certification:

• Active New York State RN License required

• Primary Source Verification of RN License required

• AHA BLS Certification required
