Surgical Care Navigator

Supporting patient engagement and outreach operations for orthopedic patients, the full-time Surgical Care Navigator will conduct health assessments, advocate for patients, and assist with care coordination in a fully remote environment. Key responsibilities Conduct outreach to potential surgical patients and administer assessments to identify care goals and barriers Act as an advocate and educator for patients, ensuring they understand their treatment plans and available resources Serve as a liaison among patients, providers, and internal teams to promote evidence-based orthopedic care Required qualifications Minimum 3 years of clinical experience in musculoskeletal, orthopedic, or value-based care preferred Experience with electronic health records and call center systems Ability to maintain HIPAA and other security and privacy guidelines Comfort with technology and rapid changes in a fast-paced environment Bilingual in Spanish and English is a plus
