Knollwood Life Plan Community | Office of People Operations | Position Description Page 1 of 4 POSITION DESCRIPTION Licensed Practical Nurse (LPN) Reports To Director of Nursing Department Nursing FLSA Status Non-Exempt — Hourly Status Full-Time and Part-Time Supervises CNAs on assigned shift Work Schedule As assigned; rotating shifts including evenings, weekends, and holidays Union Status SEIU Local 1199 Compensation This is a union-represented position with an hourly rate of $35 per hour , The Licensed Practical Nurse provides safe, resident-centered nursing care in accordance with Knollwood standards, District of Columbia Board of Nursing regulations, and the SEIU 1199 CBA. This role maintains clinical documentation, ensures medication and treatment administration accuracy, supervises assigned CNAs on shift, and promotes a respectful, therapeutic environment for all residents. The duties below are the essential functions of this position. Duties not listed are marginal and may be assigned as operational need requires; assignment of a marginal duty does not convert it into an essential function of the position. POSITION SUMMARY ESSENTIAL DUTIES AND RESPONSIBILITIES 1. Medication Administration and Treatment Administer medications and treatments per physician orders and within LPN scope. Maintain accurate and timely MARs, TARs, and incident reports. Monitor for adverse medication reactions and report to RN or provider. 2. Clinical Assessment and Documentation Document all care accurately in the EHR (MatrixCare). Monitor resident condition and report clinical changes to RN or provider. Complete shift notes and assessments before end of shift. Participate in shift handoffs and interdisciplinary care plan meetings. 3. Supervision of Nursing Assistants Supervise and support assigned CNAs on shift. Ensure care tasks are completed per assignment sheet and care plans. Provide clinical guidance and redirect as needed within scope. 4. Infection Control and Safety Comply with infection prevention, fall reduction, and safety protocols. Follow proper hand hygiene and PPE protocols. Identify and report safety hazards and environmental concerns. 5. Resident and Family Engagement Engage residents respectfully, preserving autonomy and dignity. Respond promptly to call lights and ensure resident needs are met. Communicate with families regarding resident care as appropriate. Participate in care plan reviews when assigned. 7. Professional Conduct and Compliance Maintain resident confidentiality (HIPAA compliance). Complete mandatory training on schedule. Maintain current DC LPN license throughout employment.

QUALIFICATIONS

Active LPN license in Washington, DC. Minimum 1 year of nursing experience, preferably in long-term care or geriatric settings. Proficiency in EHR systems; MatrixCare experience preferred. Strong communication, critical thinking, and family engagement skills. CPR/BLS Certification.
