Firstat RN Care Management Services is seeking an experienced Registered Nurse to join our geriatric care management team in Palm Beach County. You’ll build ongoing relationships with clients and families, coordinate care across providers, and help turn complex healthcare needs into clear, personalized plans.

What you’ll do
- Assess clients’ medical, functional, social, and emotional needs.
- Develop and update individualized care plans that reflect each client’s needs and preferences.
- Coordinate medical appointments, communicate with healthcare providers, and help oversee medication needs.
- Arrange home care and connect clients with transportation, community resources, and other support services.
- Advocate for clients and help families understand care options and next steps.
- Maintain clear documentation and keep clients, families, and care teams informed.
Requirements:
- Graduation from an accredited nursing program.
- A current Florida RN license or multistate RN license authorizing practice in Florida.
- At least two years of nursing experience mandatory.
- Previous home health nursing experience a plus.
- Strong clinical judgment, organization, critical thinking, and communication skills.
- Comfort working independently and collaborating with a team.
- A valid Florida driver’s license and automobile insurance for local travel.
- Comfort using computers, including Windows and Excel.
Previous case or care management experience is a plus, but it is not required. We provide training and support.
