Case Manager (QMHS)

Box 150 Van Wert, OH 45891 Case Manager (QMHS) Summary In this role, you’ll work directly with youth and families to provide individualized support, coordinate services, connect clients with community resources and help children and families work toward meaningful goals. You’ll also collaborate with our clinical team and community partners to provide trauma-informed care and support positive outcomes. Qualifications Include Bachelor’s degree in social work or a related human services field with an applicable Ohio license OR a high school diploma/equivalent, at least three years of relevant behavioral health or human services experience and active enrollment in a degree program leading to Ohio licensure.
