Overview Adoration Home Health and Hospice is seeking a dedicated Home Health Licensed Practical Nurse (LPN) to join our team in Jeffersonville, IN. Our LPNs provide expert, patient‑centered care and are essential for ensuring quality home health services. Responsibilities Provide 1:1 care for clients following their individual plan of care and maintaining client dignity and independence Administer medications, feedings, oxygen, and ostomy care as required per plan of care and assessment results Accurately document nursing actions, communicate with family, case manager, and physicians, and record progress toward established goals Coordinate or order supplies, medications, and feedings to ensure adequate amounts are on hand Ensure all patient needs are met in accordance with physician instructions Qualifications LPN/LPT/LVN degree from an accredited nursing program with current unrestricted license in state of practice and residence Valid driver’s license, acceptable driving record, and proof of car insurance CPR professional certification At least 1 year of nursing experience (home health experience preferred) #J-18808-Ljbffr
