Join Our Dynamic Team as a Discharge Planner! Job Description:
- Embrace the opportunity to assess, evaluate, and drive the discharge plan for patients.
- Coordinate services tailored to the appropriate level of care for each individual.
- Ensure treatment plans are initiated, evaluated, and re-evaluated daily for optimal patient outcomes.
- Monitor patient progress consistently and identify any potential barriers to discharge.
- Advocate for patient choice by adhering to Medicare regulatory guidelines, including IMM notices and MOONS.
- Work a predictable schedule: Monday-Friday, without weekend or on-call duties.
- Collaborate with an interdisciplinary team to support patient needs and deliver exceptional care.
- Enjoy a daily census of 20-25 patients, focusing on adults and geriatric populations.
- Leverage your clinical experience (3+ years) in a supportive and engaging environment-ideal for experienced travelers!
- Participate in ongoing clinical evaluations to ensure a right fit for both you and our team.
