About the Role

As a Nurse Practitioner at A Better Health Primary Care, you will play a vital role in delivering high-quality healthcare services to our patients. This position is perfect for someone with at least 2 years of Primary Care experience, who is passionate about patient care and thrives in a collaborative setting. You’ll have the opportunity to work closely with our team of dedicated professionals to promote wellness and improve the lives of those in our community.

Key Responsibilities

In this role, you’ll be responsible for: - Conducting patient assessments, including physical exams and health histories. - Diagnosing and treating acute and chronic conditions. - Developing and implementing individualized treatment plans. - Educating patients on preventative care and healthy lifestyle choices. - Collaborating with physicians and other healthcare providers to ensure comprehensive care. - Maintaining accurate, thorough and timely patient records.– Maintaining timely and comprehensive follow up care for patients (i.e. lab and test results, orders, managing various patient care documents, medication management).

What We’re Looking For

To succeed in this role, you’ll need: - A valid MS Nurse Practitioner license. - At least 2 years of experience as a Nurse Practitioner. – At least 2 years of experience in Primary Care . - Strong clinical and diagnostic skills. - Excellent communication and interpersonal abilities. - A commitment to providing compassionate, patient-centered care. -Ability to work as a team. – Ability to work independently with minimal oversight. – Ability to take care of patients and follow-up care in a timely manner.
