Geriatric Care Manager

Molina Healthcare of Illinois

Molina Healthcare is seeking a field-based care coordination professional to support LTSS and care management activities. The role requires traveling to member homes and interacting with a multidisciplinary team to ensure quality and cost-effective care across the continuum.

You will develop and monitor comprehensive care plans, coordinate waiver services, and promote integration of behavioral health with LTSS. Strong communication, organizational, and problem-solving skills are essential.
