Primary Care Nurse Practitioner (NP) – Contract Position Rural Health Clinic & Hospital-Based Care | Refugio County Memorial Hospital District Clinic Closed for Lunch: 12:00-1:00 PM Position Type: Contracted Provider Refugio County Memorial Hospital District is seeking a compassionate and experienced Nurse Practitioner (NP) to provide primary care services in our Rural Health Clinic with the opportunity to support hospital-based patient care. This position is ideal for a provider who enjoys the variety of rural healthcare—building long-term relationships with primary care patients while also working collaboratively with our hospital medical team to support continuity of care. Key Responsibilities Provide compassionate, comprehensive primary care to patients in the Rural Health Clinic. Evaluate, diagnose, and treat acute illnesses and chronic medical conditions. Perform histories and physical examinations and develop individualized treatment plans. Provide preventive care, wellness visits, screenings, and patient education. Order and interpret laboratory tests, imaging, and other diagnostic studies. Prescribe and manage medications and therapies within scope of practice. Manage chronic conditions and provide appropriate follow-up. Make appropriate referrals and coordinate care with specialists and other healthcare services. Review and address diagnostic results, medication refill requests, patient messages, and clinical tasks in a timely manner. Maintain accurate and timely documentation in the electronic health record. Collaborate with physicians, nursing staff, and other members of the healthcare team. Provide hospital-based/hospitalist support as assigned and within credentialing and privileges. Assist with evaluation and management of hospitalized patients in collaboration with the medical staff. Support continuity of care between the hospital and Rural Health Clinic, including appropriate post-hospital follow-up. Participate in admissions, rounds, discharge planning, and other inpatient responsibilities as permitted by credentialing, privileges, and the contracted scope of services. Participate in quality improvement, patient safety, and clinic/hospital initiatives. Qualifications Current and unrestricted Texas APRN license. Current national Nurse Practitioner certification appropriate to the patient populations served. Current BLS certification. DEA registration and prescriptive authority as required. Must meet hospital credentialing and privileging requirements. Primary care, family medicine, hospitalist, or inpatient experience preferred. Strong clinical judgment, communication, and organizational skills. Ability to work collaboratively across both clinic and hospital settings.
