RN Manager of Care Management
Winter Garden, Florida

The RN Manager of Care Management leads close to the work, sets the standard the team holds itself to, and keeps the department steady when volume swings. The Manager communicates and presents with the polish physicians and hospital leadership expect and delivers a consistent record of results against goals.

The Manager thrives as a self-starter, moving between the floor and the leadership table with ease, adapting quickly to change, and navigating conflict with tact and diplomacy.

Qualifications:

• Registered Nurse (RN) required
• Bachelor's Degree required
• Bachelor of Science in Nursing (BSN) preferred
• Master's Degree preferred
• Care Management leadership experience in an Acute Hospital Setting required
• Certification in Case Management (ACM/CCM) preferred

Responsibilities:

• Oversees daily operations of the Care Management department across inpatient, observation, and emergency department settings
• Carries accountability for hiring, onboarding, and retention, building a productive and engaged team
• Oversees care managers through discharge planning from admission to discharge, keeping interventions timely and preventing avoidable readmissions
• Partners with hospital leadership and executives on pace of care delays to keep progression of care and throughput moving
• Ensures documentation and compliance for discharge planning, patient choice, and related regulatory requirements
• Monitors documentation, reports, and dashboards to hold the department to standard work and quality expectations
• Leads staff participation in daily multidisciplinary rounds, takes part directly, and drives the Care Coordination improvements that come out of them
• Educates and reinforces standard work, ensuring orientation and competencies are completed
