Fairbanks Rescue Mission is a Christ-centered, faith-based nonprofit organization dedicated to demonstrating the love of Jesus Christ by providing shelter, hope, and transformational services to individuals and families experiencing homelessness. For over 75 years, we have served Interior Alaska by operating the region's only year-round emergency shelter while providing meals, case management, residential recovery support, and pathways toward stability. Position Summary

The Grant and Per Diem (GPD) Case Manager provides comprehensive case management services to veterans participating in the U.S. Department of Veterans Affairs (VA) Grant Per Diem (GPD) Program. The GPD Program is a 12-bed transitional housing program that provides veterans experiencing homelessness with intensive supportive services designed to promote housing stability, self-sufficiency, and successful transition to permanent housing.

 

Working collaboratively with veterans, VA staff, community partners, and Fairbanks Rescue Mission team members, the GPD Case Manager develops and implements individualized service plans, coordinates access to critical resources, monitors participant progress, and ensures compliance with VA GPD program requirements while supporting the Mission's commitment to compassionate, person-centered care.

Essential Duties and Responsibilities

• Conduct participant intake, assessments, eligibility determinations, and develop individualized service plans and Practical Individual Performance Plans (PIPP).
• Provide comprehensive case management focused on helping veterans obtain permanent housing, increase income, improve health, develop life skills, and achieve long-term self-sufficiency.
• Coordinate referrals to VA healthcare, behavioral health, substance use treatment, employment, education, benefits, housing programs, and other community resources.
• Meet regularly with participants to monitor progress, update service plans, provide crisis intervention, and address barriers to successful program completion.
• Assist veterans with securing and maintaining permanent housing through housing searches, landlord coordination, and connection to community resources.
• Advocate for participants while collaborating with the VA, healthcare providers, housing agencies, employers, and community partners to ensure coordinated service delivery.
• Provide transportation to appointments, interviews, housing viewings, employment activities, and other program-related services as needed.
• Maintain accurate, timely, and confidential documentation, including assessments, progress notes, HMIS data entry, discharge summaries, and all required VA GPD records.
• Monitor participant outcomes and assist with grant reporting, audits, program monitoring, and continuous quality improvement while ensuring compliance with VA, HIPAA, and Fairbanks Rescue Mission policies.
• Participate in staff meetings, trainings, and other duties as assigned to support the success of the GPD Program and the Mission.

Qualifications

• Bachelor's degree in Social Work, Psychology, Counseling, Human Services, or a related field preferred; equivalent education and relevant experience may be considered.
• Minimum of two years of case management experience, preferably working with veterans, individuals experiencing homelessness, or other vulnerable populations.
• Knowledge of homelessness, behavioral health, substance use disorders, trauma-informed care, and veteran services.
• Strong assessment, communication, organizational, documentation, and problem-solving skills.
• Ability to work independently while collaborating effectively with multidisciplinary teams and community partners.
• Valid Alaska driver's license with an acceptable driving record and the ability to transport participants safely.
• Proficiency with Microsoft Office and electronic case management systems; HMIS experience preferred.

• Ability to establish professional, supportive relationships while maintaining appropriate boundaries.
• Strong crisis intervention, conflict resolution, and advocacy skills.
• Ability to maintain confidentiality and comply with HIPAA, VA regulations, and agency policies.
• Excellent organizational skills with the ability to manage multiple priorities in a fast-paced environment.
• Demonstrates professionalism, integrity, compassion, cultural sensitivity, and sound judgment.

Physical Requirements

• Ability to sit, stand, walk, bend, stoop, and lift up to 25 pounds.
• Ability to safely operate a motor vehicle and transport participants throughout the community.
• Ability to respond appropriately during crisis situations.
• Frequent local travel and occasional evening or weekend work may be required to meet participant needs.

Work Environment

• Office-based position located at Fairbanks Rescue Mission with regular interaction with program participants, staff, community partners, and the Department of Veterans Affairs.
• Standard business hours (8:00 a.m.–4:00 p.m. or 9:00 a.m.–5:00 p.m.), with occasional evening or weekend meetings, trainings, or events as needed.
• Occasional local travel required to attend meetings, trainings, partner agencies, and VA functions.
