Boston Medical Center (BMC) is seeking a Senior Housing & Community Support Specialist to lead housing-focused case management for high-risk patients experiencing homelessness. The role includes ownership of special projects, collaboration with interdisciplinary teams, and direct in-home/community support to help patients obtain and sustain housing. The SHCS Specialist will engage patients, coordinate with housing providers, and guide them through housing processes, while mentoring junior staff #J-18808-Ljbffr Boston Medical Center (BMC)
