We are seeking an experienced Intake Coordinator to join our growing team and be the first point of contact for patients entering our care. Key Responsibilities Manage incoming referrals and initiate the intake process Verify insurance and obtain authorizations Communicate with physicians, hospitals, and case managers Enter patient and physician information into Kinnser (WellSky) Work closely with the Patient Care Coordinator to ensure staffing needs are met Coordinate with clinical staff to ensure timely admissions Ensure all documentation meets Medicare and agency standards Required to be ON-Call on a rotating shift Qualifications 1–2 years of intake experience in home health care Strong working knowledge of AIDEN/AIDA and Kinnser/WellSky Excellent communication and organizational skills Ability to manage multiple tasks in a fast-paced environment Bilingual a plus Why Join Us?
