Case Management Registered Nurse
Location: Overland Park, KS
Assignment Duration: This is a 13-week contract position.
Schedule: Monday to Friday, 8-hour shifts (8A - 5P)
Job Description: We are seeking a compassionate and dedicated Case Management Registered Nurse to join our team. The ideal candidate will have expertise in discharge planning and observation management, with a focus on providing exceptional patient care. You will collaborate closely with multidisciplinary teams to ensure smooth transitions and continuity of care.
Responsibilities:
- Conduct assessments for discharge planning and patient needs.
- Manage observation cases effectively following established protocols.
- Coordinate care with healthcare teams and community resources.
- Document patient progress and care plans in Midas, Meditech, and Well Sky.
- Ensure compliance with interqual criteria and hospital policies.
Required Experience / Certifications / Licensure:
- Current RN license in the state of practice.
- Experience in case management and discharge planning is essential.
- Advanced certifications in case management preferred.
