Contract | Full-Time | Onsite | Day Shift (Monday–Friday, 7:00 AM–3:00 PM or 8:00 AM–4:00 PM, flexible) | 32 Guaranteed Hours Per Week

Key Responsibilities

• Provide occupational therapy services to patients in skilled nursing facility (SNF) and home health settings
• Implement treatment plans developed by a supervising Occupational Therapist
• Meet productivity expectations of 87% in the SNF setting and 95% in the home health setting
• Document patient progress and treatment notes using Net Health and Home Care Home Base software
• Collaborate with an onsite Therapy Supervisor to ensure quality patient care

Required Qualifications

• Current COTA certification in good standing
• Active state licensure or eligibility to practice in Minnesota
• Ability to work across SNF and home health settings
• Proficiency or ability to learn electronic documentation platforms

Preferred Experience / Skills

• Experience with Net Health and/or Home Care Home Base software
• Prior experience in both SNF and home health environments
• New graduates are welcome to apply
