As an Occupational Therapist, you will evaluate patients to determine if therapy can alleviate pain, improve function, or enhance quality of life. Your role involves collaborating closely with nursing staff to identify patients who have experienced falls or functional decline, ensuring timely and effective intervention.

Key Responsibilities:

• Conduct quarterly screenings for all patients, including new admissions and re-admissions.
• Utilize SMART tools to document and print screening forms accurately.
• Schedule and perform patient assessments, reviewing medical charts to understand prior and current functional levels.
• Perform visual assessments and engage patients to discuss their concerns and complaints.
• Gather insights from nursing staff regarding patient functionality and challenges.
• Determine the necessity for therapy services or environmental adjustments based on screening outcomes.
• Provide recommendations to the therapy team to optimize patient care plans.
• Monitor patients not requiring immediate therapy to ensure they maintain their highest functional level through regular follow-ups.

Education: A degree in Occupational Therapy from an accredited program is required, along with current licensure as an OTR.

Experience: Prior experience in patient screening and therapy assessment is preferred, with strong communication skills to collaborate effectively with interdisciplinary teams.

Requirements: Ability to perform detailed patient evaluations, maintain accurate documentation, and provide compassionate care tailored to individual patient needs.
